Hydromorphone(Dilaudid 8 mg) was first synthesized and researched in Germany in 1924 by Knoll. The brand name Dilaudid is more widely known than the generic term hydromorphone and, because of this, Dilaudid is often used generically to mean any form of hydromorphone.
Dilaudid is a derivative of morphine. This is actually Hydromorphone(dihydromorphinone) sold under the brand name Dilaudid among others, is a centrally acting pain medication of the opioid class. Hydromorphone is to morphine as hydrocodone is to codeine. Dilaudid is a narcotic. Hydromorphone(Dilaudid) is bitter and hydrophilic like most opiates, not lipophilic, so it is absorbed poorly and slowly through mouth membranes. It is more soluble in water than morphine.
Small amounts of hydromorphone are detected in trace amounts of opium on rare occasions. It appears to be produced by the plant under circumstances and by processes which are not understood at this time and may include the action of bacteria.
Dilaudid is commonly used in the hospital setting, mostly intravenously because its bioavailability is very low orally, rectally, and intranasally. It is administered under the tongue than to swallowing for bioavailability and effects. Dilaudid(Hydromorphone) causes constipation by reducing gut motility because it acts on the myenteric plexus in the intestinal tract. Morphine decreases the rate of intestinal transit. Dilaudid, like other opioids, often causes hypogonadism and hormone imbalances in chronic users of both sexes. This side effect is dose-dependent and occurs in both therapeutic and recreational users. A high dose of morphine can impair finger tapping and the ability to maintain a low constant level of isometric force.
